Lake County Resource Guide is a detailed website containing links to information about services, programs, and resources in Lake County. 

Presumptive Transfer

All presumptive transfer requests can be uploaded by completing our Presumptive Transfer Form. Communication can be emailed to Lake County Behavioral Health's contact or by calling 707-274-9101 and asking to speak with the Managed Care Coordinator.

Lake County Behavioral Health Services clients have many rights, including the right to report issues about the services they receive.
